Voor een goede behandeling is het noodzakelijk dat ik, als je behandelend therapeut, een dossier met jouw gegevens aanleg. Dit is ook wettelijke plicht opgelegd door de WGBO. Je dossier bevat aantekeningen over je gezondheidstoestand, je\u00a0 persoonlijke gegevens als naam, adres, e.d., en gegevens over uitgevoerde onderzoeken en behandelingen.<\/p>

Also included in the file are data necessary for your treatment that I have requested from another healthcare provider, e.g. the GP, after your explicit consent.<\/p>

Ik doe mijn uiterste best om je privacy te waarborgen. Dit betekent dat onder meer dat ik:<\/span><\/p>

  • zorgvuldig om ga met jouw persoonlijke en medische gegevens: deze bewaar ik in een beveiligd systeem<\/li>
  • er voor zorg dat onbevoegden geen toegang hebben tot je gegevens. Als je behandelend therapeut ben ik de enige die toegang heeft tot de gegevens in je dossier. Ik ben gebonden aan de wettelijke geheimhoudingsplicht (beroepsgeheim)<\/li><\/ul>

    • Om andere zorgverleners te informeren, bijvoorbeeld bij verwijzing naar een andere behandelaar. Dit gebeurt alleen met je expliciete schriftelijke toestemming<\/li>
    • For use by a substitute therapist during absence<\/li>
    • Voor het genanonimiseerd gebruik tijdens intervisie of supervisie met collega’s<\/li>
    • Een klein deel van de gegevens uit je dossier wordt gebruikt voor de financi\u00eble administratie, zodat ik of mijn administrateur een factuur\/zorgnota kan opstellen. Dit houdt vaak in: naam, adres, woonplaats, geboortedatum, datum van behandeling, korte omschrijving (zoals ‘psychosociaal consult’), kosten van het consult<\/li><\/ul>
